JOB DESCRIPTION
HealthPartners is looking for an exceptional, patient focused Advanced Practice Provider to join our team and make a difference for our patients and our community. This Advanced Practice Provider will support clinic visits for our Specialty Center at the Phalen Blvd (St. Paul) location, featuring our digestive care/gastroenterology practice.
GI Advanced Practice Providers manage a full patient caseload and are trusted experts in this specialized field. They provide new patient assessment for patients with liver and general GI disease; ongoing chronic disease management for this patient population; and between-visit partnership with patients in the management of their disease. These providers are an integral part of the GI team and the patient care they provide is foundational and essential in caring for the HealthPartners patient population with GI and liver disease.

Required Qualifications:

Successful completion of an approved Nurse Practitioner program or accredited Physician Assistant program.

Licensed as a Physician Assistant OR currently licensed as an APRN in the state of MN and current MN RN license

Current APP certification as Physician’s Assistant OR current APRN Specialty Certification

For PA, must be approved by Minnesota Board of Medical Practice for prescription writing privileges

Current BLS

Current DEA certification

Must maintain CME requirements

1-year experience as a NP or PA

POSITION SUMMARY STATEMENT:
Regions Digestive Care is staffed by Advanced Practice Providers who are dedicated to clinic and hospital service, as well as GI physicians who have varying clinic rotations along with their procedural, and hospital schedules. GI Advanced Practice Providers manage a full patient caseload and are trusted experts in this specialized field. They provide new patient assessment for patients with liver and general GI disease; ongoing chronic disease management for this patient population; and between-visit partnership with patients in the management of their disease. These providers are an integral part of the GI team and the patient care they provide is foundational and essential in caring for the HealthPartners patient population with GI and liver disease. Performs other duties as assigned.

Coordinates flow of Inpatient Service.

Develops and reviews work plan for the day with rounding physician partner.

Communicates regularly with partner Inpatient APP to coordinate consult services and to balance workloads.

Discusses and develops diagnosis and treatment plan with staff gastroenterologist.

- Assesses and completes H&P on new consults as selected/assigned.
- Orders diagnostic testing as indicated.
- Evaluates treatment for assigned patients and communicates information to physician partner.
- Designs follow-up plan for patients to be seen in the clinic when discharged from the hospital.
- Documents assessment, treatment, evaluation, and follow-up plan in the electronic medical record.

Discusses with and instructs patient/family members regarding treatment and follow-up.

PHYSICAL REQUIREMENTS:
Please list any physical requirements necessary to the performance of this job (e.g. lifting, carrying, stretching, sitting or standing for long periods of time). Please specify details such as number of pounds expected to lift, number of hours must be on feet, etc.
Stands and walks up to 90% of shift while delivering patient care and treatment activities. Bends, reaches, twists and stoops frequently while performing these same activities. Performs seated or standing work at a computer screen up to 50% of shift. Uses hands and fingers daily while using equipment, performing treatments and keyboarding. Pushing and/or pulling may be required when transporting patients and equipment, performing CPR, or when intervening with patients. Low level work can occur when performing CPR or wheelchair footrest adjustments.
