Job Title: Physician Assistant (PA) – Senior Living Rounding | Wound Care & Chronic Care Management.

Company: Twin Cities Physicians.

Compensation: $105,000 to $120,000 annually + autonomy + clinical growth.

Benefits: Full Benefits Package.

Location: Greater Twin Cities Metro Area.

Schedule: Full-Time | Mobile Rounding Across Communities.

Position Summary:

We are seeking a Wound Care focused Physician Assistant (PA-C) to provide on-site rounding, advanced wound management, and comprehensive geriatric care. In this role, you will manage complex patient populations, lead care planning, and collaborate closely with interdisciplinary teams. This is an opportunity to expand your clinical expertise, influence care delivery, and play a direct role in improving patient outcomes across multiple senior living settings.

Responsibilities:

  • Provide on-site rounding, primary care, and advanced wound care services.
  • Manage complex wounds including pressure injuries, diabetic ulcers, and post-surgical cases.
  • Drive chronic care management, preventative care, and clinical coordination.
  • Collaborate with physicians, nurses, therapists, and facility staff.
  • Reduce hospitalizations through proactive and consistent clinical oversight.
  • Communicate with families, caregivers, and interdisciplinary teams.
  • Complete documentation, medication management, and lab coordination.

Qualifications:

  • Active Minnesota Physician Assistant (PA-C) license in good standing.
  • Experience in wound care, geriatrics, primary care, or post-acute settings preferred.
  • Strong clinical judgment and ability to manage complex cases independently.
  • Valid driver’s license and reliable transportation.
  • Commitment to patient-centered, outcomes-driven senior care.

Military Occupational Classification (MOC) Codes:

  • Army: 65D – Physician Assistant.
  • Navy: Physician Assistant (230X).
  • Air Force: 42G3 – Physician Assistant.
  • Marines: Physician Assistant (230X).
  • Coast Guard: Health Services Officer (HSO) / Physician Assistant.
