Physician Assistant or Nurse Practitioner-Primary Care Provider

Overview:

Family Centers seeks an experienced Physician Assistant (Primary Care) or Nurse Practitioner (FNP or PNP) to deliver comprehensive primary medical care to residents in an outpatient pediatric clinic in Greenwich, CT

The Role:

The Physician Assistant (Primary Care) or Nurse Practitioner (FNP or PNP will be responsible for intake assessments of residents, diagnosis and treatment of common and chronic illnesses, referrals, vaccines, screenings, promoting optimal health and preventative care. The Primary Care Provider will collaborate with interdisciplinary program team members, be responsible for the medication management and medical case management of the residents. The schedule is a flexible one that will require some evenings and weekend hours.

Education and Experience:

  • Master’s or post-Graduate’s degree from an accredited program
  • Current active license to practice in the State of Connecticut
  • Minimum of 2+ years related experience and/or training is preferred
  • Demonstrated aptitude in providing medical services to under-served populations.
  • Experience in treating the Pediatric population is preferred
  • Excellent multitasker, leadership, and team oriented
  • Bilingual (Spanish) is required.
  • Medical malpractice insurance (in addition to that offered by the agency)
  • Current active DEA license
  • Infectious Control Certified a plus
