Full Job Description
Position Summary:
The Nurse Practitioner (NP) or Physician Assistant (PA) is a licensed and credentialed advanced practice provider who delivers high-quality, compassionate, and evidence-based care under the supervision of a physician, consistent with scope-of-practice regulations. The NP/PA provides comprehensive diagnostic and therapeutic care, health promotion, and disease prevention to patients across the lifespan.
Essential Duties and Responsibilities:
- Perform comprehensive and focused physical exams, histories, and evaluations
- Diagnose and manage acute and chronic conditions in collaboration with supervising physicians
- Order and interpret diagnostic tests (labs, imaging, etc.)
- Develop and implement patient-centered treatment plans
- Prescribe medications and other therapies in accordance with state and federal regulations
- Educate patients and families on diagnoses, treatment options, preventive health, and wellness
- Collaborate with multidisciplinary teams to coordinate patient care
- Provide follow-up care and assess response to treatments
- Accurately and thoroughly document all patient interactions in the electronic medical record (EMR)
- Participate in quality improvement initiatives, clinical meetings, and continuing education
- Adhere to clinical protocols, compliance standards, and organizational policies
Qualifications:
- Graduate of an accredited Nurse Practitioner or Physician Assistant program
- Current and unrestricted state licensure as a Nurse Practitioner (NP) or Physician Assistant (PA-C)
- National board certification (AANP, ANCC, or NCCPA)
- Current DEA license and prescriptive authority (or eligibility)
- BLS and ACLS certification (as required by setting)
- Proficient with EMR systems and clinical documentation
- Fluent in Spanish

Work Environment:
- This position offers a hybrid and flexible work environment, combining both in-clinic and remote telehealth responsibilities. The provider will have the opportunity to see patients in person at designated clinical locations while also conducting virtual visits from a secure off-site location.

- Requires use of a secure, HIPAA-compliant electronic medical record (EMR) system for documentation and communication
