Looking for a Nurse Practitioner to join our pediatric and adult palliative teams in to assist our Medical Director in providing oversight of care for hospice patients. Provide education and collaborate with clinical staff and complete Face-to-Face evaluations of patients who require re-certification on the Medicare Benefit. JOB DETAILS: In collaboration with the Medical Director, the NP will complete Face-to-Face evals of patients requiring Medicare recertification & to discharge patients. Communicates to Case Manager and/or Clinical Supervisor upon patient’s change in condition, ensuring follow up. Charting will be completed in accordance with policies and procedures. Visit the general ICU to provide medical services to inpatients including pain & symptom management, education to patient & family, provide support to staff. May provide home visits to patients in Skilled Nursing Facilities, Residential Care Facilities and private homes. Participate in Interdisciplinary Team meetings May participate in the care of patients enrolled in our Palliative Care Program. May participate/provide education to Clinical Team members EXPEREINCE: Experience in the hospice care setting and knowledge of Hospice regulations. A minimum of two (2) years of hospice/palliative care experience. LICENSE/CERTIFICATIONS: Possess reliable transportation, which may be used in the course of work, and have a valid California driver’s license with acceptable DMV driving record. Proof of auto insurance and current auto registration. Current BLS License.
