Full Job Description
PRIMARY RESPONSIBILITIES
- Conduct comprehensive patient evaluations, follow-up visits, and chronic disease management
- Perform cardiovascular risk assessments and implement guideline-directed therapy
- Develop and update treatment plans in collaboration with the supervising cardiologist
- Provide patient education on cardiovascular health, medications, and lifestyle modification
- Coordinate care with primary care providers and referring physicians
- Maintain accurate, timely, and compliant clinical documentation
- Support practice operations and contribute to a positive, team-oriented environment
Pay: $80,000.00 – $120,000.00 per year
