Full Job Description
TSF Medical Services is seeking a motivated and compassionate Nurse Practitioner to join our growing skilled nursing facility team in the Brevard County area.
This position offers flexibility ranging from part-time to full-time and is ideal for an NP who enjoys autonomy, continuity of care, and building meaningful relationships with patients and facility staff.
Responsibilities
- Provide medical management and follow-up care for patients in skilled nursing and rehabilitation settings
- Perform assessments, admissions, progress notes, and discharge planning
- Collaborate closely with physicians, nursing staff, therapy teams, and administration
- Manage chronic and acute medical conditions in post-acute and long-term care patients
- Participate in quality improvement and care coordination initiatives
Qualifications
- Licensed Nurse Practitioner in the state of Florida
- Adult, Family, or Gerontology certification preferred
- Skilled nursing, internal medicine, hospital, or geriatrics experience preferred but not required
- Strong communication and documentation skills
- Ability to work independently and efficiently
