Nurse Practitioner (NP) / Physician Assistant (PA) – Mobile Primary Care
Mobile MD, PA – McKinney, TX
Mobile MD is seeking a compassionate and motivated Nurse Practitioner or Physician Assistant to join our growing mobile medical practice serving homebound patients in:
- Assisted Living (AL)
- Memory Care (MC)
- Independent Living (IL)
- Private Homes
This position offers the opportunity to build meaningful relationships with patients while practicing highly autonomous medicine in a supportive team environment.

Responsibilities
- Provide primary care visits in residential and facility settings
- Assess, diagnose, and manage chronic and acute conditions
- Coordinate care with families, facility staff, specialists, and home health/hospice
- Perform medication management and follow-up visits
- Maintain accurate documentation in EMR
Qualifications
- Licensed NP or PA in Texas
- DEA license
- Experience in geriatrics, internal medicine, family practice, SNF, home health, palliative care, or mobile medicine preferred
- Comfortable working independently
- Strong communication and organizational skills
- Reliable transportation required
