Overview
Join our dynamic healthcare team as a Family Nurse Practitioner, where your expertise will directly impact patient care across diverse populations. In this vital role, you will provide comprehensive primary care services, perform advanced clinical assessments, and develop personalized treatment plans. Duties

  • Conduct thorough physical examinations, vital sign assessments, and detailed patient histories to inform diagnosis and treatment strategies.
  • Manage a broad spectrum of medical conditions across age groups, including , geriatrics,and adult patients, utilizing your expertise in family medicine.
  • Administer injections, IV infusions, suturing to enhance patient well-being.
  • Perform diagnostic evaluations using laboratory specimen collection and processing, and medical imaging techniques such as radiography or ultrasound.
  • Coordinate with multidisciplinary teams for discharge planning, case management, utilization review, and telehealth consultations to ensure seamless patient care transitions.
  • Document all patient interactions accurately within Electronic Health Record (EHR) systems like eClinicalWorks while adhering to HIPAA regulations.
  • Provide health coaching on nutrition, chronic disease management, memory care, hospice & palliative medicine, and behavioral health to empower patients in their health journeys.

Requirements

  • Valid Family Nurse Practitioner certification with current state licensure.
  • Proven experience in primary care settings with proficiency in medical office operations and EMR/EHR systems such as eClinicals.
  • Demonstrated skills in acute pain management, infection control protocols, and emergency procedures including airway management and ventilator support.
  • Experience working with diverse populations including seniors in assisted living or nursing homes, individuals with developmental disabilities or memory care needs.
  • Knowledge of CPT coding, ICD-10/ICD-9 coding standards for accurate documentation and billing processes.
  • Strong clinical research background or participation in teaching hospital programs is a plus. Pay: $84,739.73 – $98,181.19 per year
