**Requirements**
Nurse Practitioners who have graduated from an accredited Certified Acute Care or Family Nurse Practitioner program. Physician Assistants who have graduated from an accredited Physician Assistant program with a Masters Degree in Physician Assistant Certification. The candidate must have a current State of Illinois Nurse Practitioner or Physician Assistant License, DEA License, and BLS certification. Experience with EPIC EMR is preferred.
**Description**
Hamilton Memorial Hospital District Family Clinic seeks an Advanced Practice Provider (APP) for its Rural Health Clinics in McLeansboro and Carmi, Illinois. HMHD Clinics are powered by two physicians and five APPs, focusing on our mission of enhancing healthcare delivery to our community and the surrounding area by providing accessible, quality healthcare services, education, and facilities.
Job Overview: The APP may be a PA or NP who has been prepared with the appropriate graduate-level education and is authorized to practice and prescribe as such.
Position Summary: As an APP, you will play a crucial role in our healthcare team, providing primary care to patients in collaboration with physicians in the clinic. Your expertise and collaboration will be vital in following established standards and practices. You will be expected to care for patients in the following age-specific categories: Pediatric, Adolescent, Adult, and Geriatric.
