Full Job Description
Responsibilities:
- Provide on-going clinical support, coordination and management for patients in a healthcare environment.
- Manage patient care across multiple providers.
- Collaborate with other health care providers to ensure the delivery of quality, cost-effective patient care.
- Provide care coordination and consultation services.
- Make clinical decisions using judgment and experience.
- Participate in development of new treatment protocols and systems.
- Provide leadership in quality and safety initiatives.
- Facilitate information exchange between patient care sites.
- Diagnose and treat acute chronic illness.
Job Type: Full-time
Pay: $113,000.00 – $120,000.00 per year
