We complete prompt evaluations to address patients’ needs through proactive and quality healthcare in the comfort of their home.
We are looking to hire NPs or PAs who can conduct a thorough and precise health assessment of patient condition, functional abilities, home bound status, and environment and social factors. The NP/PA will go to patients’ homes to develop individualized plans of care for each patient and should be able to make appropriate referrals for additional services such as PT, OT, MSW, HHA. They should be able to identify changes in patient conditions, consult with patient care team and implement appropriate interventions. The following responsibilities include, but are not limited to:
- Keep prompt communication with office staff (confirming appointments and sending in completed forms)
- Provide admission forms and progress notes within 24hours of initial patient visits
- Review patient information as needed (H&P, Med list, etc.)
- Communicate professionally with patient and/or patient family before and throughout patient visits.
Job requirements:
- Must possess an active, unrestricted FL License
- Current BLS/ACLS
- Reliable Transportation/willing to travel within the borough
- Must have your own malpractice insurance. (Can connect you with a great broker if needed)
- Experience preferred, but will consider motivated new grad
- Spanish speaking preferred, not required
This is a Part-time/per diem position, 1099 contract. Schedules are flexible around your availability.
