Physician Assistant for Skilled Nursing Facility

Alertive Health is seeking a dedicated and experienced Physician Assistant (PA) to join our growing team in Riverside, California. This role offers the opportunity to work across many skilled nursing facilities providing care to a diverse patient population on rotating day and night and weekend shifts.

We’re looking for providers who are compassionate, confident, and committed to delivering high-quality care in both acute and post-acute environments.

Responsibilities:

· Perform patient assessments, diagnose medical conditions, and implement treatment plans

· Provide medical management for patients in a SNF settings

· Conduct daily rounds and coordinate continuity of care

· Collaborate with physicians, nurses, case managers, and facility staff

· Order and interpret lab work and diagnostic tests

· Prescribe medications and provide patient education

· Accurately document encounters and maintain EHR compliance

· Respond to urgent care needs and patient changes promptly

Qualifications:

· Active Nurse Practitioner license in California (FNP or AGNP preferred)

· Minimum of 1 year of experience in a SNF environments

· Strong diagnostic and clinical decision-making skills

· Familiarity with electronic health records (EHR)

· Excellent communication and teamwork abilities

· Willingness to work rotating shifts, including nights, weekends, and holidays
