Duties

• Conduct in-home patient assessments and physical examinations, tailoring care to the patient’s environment.
• Diagnose and manage acute and chronic illnesses with an emphasis on convenience and accessibility.
• Develop and implement individualized treatment plans that align with patient needs and home settings.
• Provide health education and coaching to patients and their families in an approachable and practical manner.
• Collaborate effectively with physicians, nurses, and other healthcare team members to deliver cohesive care.
• Perform basic in-home procedures and ensure patients have necessary follow-up care.
• Maintain accurate, detailed patient records, adhering to healthcare regulations and standards.
• Travel to patients’ homes and other community settings as needed, managing time and schedules efficiently.
