Full Job Description

Overview:
Community Health Partners is seeking an Advance Practice Provider (APP) to work in an established Oncology practice located in Clovis. The APP provides routine care and management of the patient with acute and chronic complaints, in collaboration with the supervising physician, and through implementation of standardized procedures. Responsibilities include complete/comprehensive history and physical examination, ordering and interpretation of diagnostic studies, and prescribing appropriate treatments in collaboration with the attending physician. Other responsibilities include consultation with specialist physicians and other team members regarding findings/changes in patient condition, initiation and adjustment of chemotherapy orders in collaboration with the physician, documentation of patient care/procedures per policy, and ensuring longitudinal communications with primary oncologist and referring providers regarding patient status.

Responsibilities:In this role, you will:
Provide comprehensive assessments, perform medical diagnosis, write prescriptions and formulate treatment plans.
Diagnose medical conditions, plan and implement interventions, ensure patient safety, and contribute to quality and regulatory activities.
Document and maintain precise and complete records of patients.

Qualifications:Experience and Education Minimum Requirements:
Completion of a Nurse Practitioner or Physician Assistant program required
Credentialed by the CMC Medical Staff Office as an Allied Health Professional and privileged to perform Nurse Practitioner or Physician Assistant Standardized Procedures required
2 years of clinical experience in the area of specialty required
If qualifying as a Nurse Practitioner, the following is required
NP – Current State of California Nurse Practitioner License required
NPF – Nurse Practitioner Furnishing Number required.
If qualifying as a Physician Assistant, the following is required
PA – State of California Physician Assistant License required
DEA – DEA Controlled Substance Number required
BLS – Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required
Ability to participate in a federally funded program (Medicare and Medicaid) required
NPC – National Certified Nurse Practitioner by ANCC (American Nurses Credentialing Center) or AANP (American Academy of Nurse Practitioners) preferred
