Altos Care brings medical care directly to seniors where they live — in skilled nursing facilities, assisted living communities, board and care homes, and group homes across the Bay Area. Instead of asking older adults to travel to a clinic, our clinical team meets them in their communities, delivering consistent, proactive, and coordinated care.
Many of the seniors we serve manage multiple chronic conditions, and a number live with untreated mental health needs. Our model pairs in-person provider visits with continuous monitoring and early intervention, helping to prevent avoidable hospitalizations and keep patients healthier and more comfortable in their place of living.
About the Role
We are looking for a part-time Nurse Practitioner or Physician Assistant to provide on-site primary care to seniors primarily in a skilled nursing facilities in Oakland/Berkeley area.
In this role, you will:
- Round on skilled nursing facility (SNF) patients in the Oakland / Berkeley area, managing post-acute and short-term rehabilitation patients through their recovery
- Perform admission, follow-up, and discharge evaluations and manage medications for SNF residents
- Provide Telehealth care to patients residing in board and care and assisted living facilities across the Bay Area
- Make in-person visits to board and care and assisted living residents when a patient’s condition calls for a hands-on evaluation
- Perform primary care evaluations, diagnosis, and medication management
- Manage chronic conditions such as diabetes, heart disease, COPD, and hypertension
- Coordinate care with specialists, facility staff, and patients’ existing physicians
What We’re Looking For
- Active California license as a Nurse Practitioner (NP) or Physician Assistant (PA)
- Current DEA registration (or eligibility to obtain)
- Experience in geriatrics, primary care, skilled nursing, home-based, or facility-based care preferred
- Comfort working independently and traveling between facilities in the Oakland / Berkeley area and greater Bay Area
- Reliable transportation and a valid California driver’s license
- A patient-centered approach and strong communication with facility staff and families
- Compassion for and commitment to serving an older adult population
Pay: $75.00 – $100.00 per hour
