Position Overview
We are seeking an experienced and motivated Nurse Practitioner to join our mobile wound care team serving East Phoenix, Scottsdale, and surrounding areas. This role offers significant autonomy, a consistent weekday schedule, and the opportunity to make a meaningful difference in patient recovery and quality of life.
Key Responsibilities
- Perform comprehensive wound assessments and develop individualized treatment plans
- Provide hands-on wound care in home and facility settings
- Collaborate with interdisciplinary teams while working independently in the field
- Conduct telehealth visits as permitted by state regulations
- Document encounters accurately and efficiently
- Prescribe medications and treatments when clinically appropriate
- Travel locally to patient locations (mileage reimbursed)
- Optional: Participate in SNF or rehab facility rounding
Qualifications
- Active Arizona Nurse Practitioner license + board certification (FNP, APRN)
- Preferred: Experience in wound care, primary care, or urgent care
- Bonus: ER, ICU, or other critical care background
- Strong clinical judgment and efficient documentation skills
- Excellent communication and patient engagement abilities
- Self-motivated, organized, and comfortable working independently

Pay: $120,000.00 – $145,000.00 per year
