Job Summary
AxioGraft Wound Care Medical Group is seeking a dedicated and compassionate Nurse Practitioner (NP) or Physician Assistant (PA) to join our growing team specializing in advanced wound care and regenerative medicine.

The provider will perform comprehensive wound assessments, develop and implement treatment plans, and deliver bedside procedures including debridement and application of advanced biologic grafts. Services are provided in collaboration with home health agencies and skilled nursing facilities, with physician support available via telemedicine.

Pay is based on per patient basis raging from $150-$165 per visit

Duties

  • Conduct thorough wound assessments, including evaluating wound etiology, size, depth, and infection status using clinical judgment and diagnostic tools such as sonography when appropriate.
  • Develop and implement individualized treatment plans incorporating advanced wound care techniques such as debridement, dressing selection, compression therapy, and skin grafting procedures.
  • Administer specialized treatments including Botulinum toxin injections for pain management or spasticity control when indicated.
  • Manage complex cases involving diabetic foot ulcers, pressure injuries, surgical wounds, or traumatic injuries in outpatient clinics, hospitals, or long-term care facilities.
  • Collaborate with multidisciplinary teams to coordinate patient care plans, discharge planning, and case management for optimal healing outcomes.
  • Utilize electronic health record (EHR) systems like Epic or eClinicalWorks to document patient encounters accurately and maintain comprehensive medical records in compliance with HIPAA regulations.
  • Educate patients and caregivers on wound prevention strategies, proper dressing techniques, nutrition support, and lifestyle modifications to promote healing and prevent recurrence.

Requirements

  • Valid Nurse Practitioner or Physician Assistant license.
  • Proficiency with EMR/EHR systems for documentation and order entry.
  • Strong knowledge of anatomy, physiology, infection control protocols, and diagnostic evaluation techniques including laboratory testing and medical imaging.
  • Experience working with diverse populations including geriatrics (memory care), individuals with disabilities or developmental disabilities.
  • Ability to perform physical examinations efficiently while demonstrating excellent patient assessment skills.
  • Knowledge of Workers’ Compensation law and post-acute care processes is a plus.
