Position
Primary Care & Urgent Care Provider (Nurse Practitioner or Physician Assistant)
Location: Florence & Manchester, Los Angeles, CA
Schedule: Monday–Friday, 10:00 AM–5:00 PM
Employment Type: Independent Contractor (1099)
Compensation: $70.00/hour
Summary
Provide primary and urgent care services to approximately 10–15 patients daily within a recuperative care clinic serving individuals experiencing homelessness and complex medical, behavioral health, and social needs.
Responsibilities
- Evaluate and treat acute and chronic conditions.
- Manage diabetes, hypertension, COPD, CHF, CKD and other chronic illnesses.
- Perform medication reconciliation.
- Provide patient education and coordinate specialty, behavioral health, home health and community referrals.
- Collaborate with case managers, psychiatry and interdisciplinary staff.
- Participate in quality improvement initiatives.
Documentation
Complete all EMR documentation within 24 hours of each patient encounter. Maintain HIPAA compliance and timely review of labs, imaging and refill requests.
Qualifications
Current California NP or PA license, DEA, BLS, minimum two years of primary care, family medicine, internal medicine, urgent care or community health experience. Experience with underserved populations preferred. Spanish preferred.
Pay: $70.00 per hour
