Nurse Practitioner / Physician Assistant — Primary Care SNFist

The Opportunity
We’re looking for a Nurse Practitioner or Physician Assistant who thrives in an autonomous, fast-paced clinical environment and genuinely enjoys the complexity of the post-acute patient. This is a SNFist-model role, meaning you’ll carry your own panel of patients across one or more skilled nursing facilities, managing everything from admission assessments and chronic disease management to acute illness and care transitions.

Patient Care and Rounding — Manage acute and chronic conditions, write orders, and guide individualized care plans for a post-acute patient panel across assigned SNF locations.
Admissions and Transitions — Lead SNF admissions and coordinate hospital-to-SNF care transitions in partnership with discharge planners.
Chronic Disease Management — Manage complex, multi-morbidity patients including CHF, COPD, diabetes, renal disease, and dementia with evidence-based care plans.
Collaborative Practice — Partner with physicians, nursing staff, social workers, therapists, and consulting specialists to deliver coordinated, whole-patient care.
Documentation and Compliance — Maintain timely, accurate clinical documentation and participate in quality improvement initiatives and facility care conferences.
QUALIFICATIONS
Education — Master’s degree or higher in Nursing (NP) or completion of an accredited PA program. Board certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Primary Care NP (AGPCNP), or PA-C required.
Licensure — Active, unrestricted California NP or PA license and current DEA registration. APRN certification where applicable.

Experience — Minimum of 6 months of clinical experience as an NP or PA. Prior acute care, hospital, or post-acute/SNF experience strongly preferred.
Clinical Acumen — Comfortable managing medically complex, elderly patients with multiple comorbidities and strong clinical decision-making skills.
Communication — Confident and compassionate communicator with patients, families, facility staff, and interdisciplinary team members.
Autonomy — Self-directed and organized, with the ability to manage a patient panel independently while knowing when to escalate.
Commitment to Patient-Centered Care — Demonstrated dedication to delivering dignified, high-quality care to the geriatric and post-acute population.
