Join our dynamic healthcare team as a Hospitalist Nurse Practitioner and SNFist, where you will play a vital role in delivering comprehensive inpatient care across diverse medical settings and follow patients in SNF. This energetic and rewarding position offers the opportunity to collaborate with multidisciplinary teams, utilize advanced clinical skills, and positively impact patient outcomes. You will be at the forefront of acute and chronic disease management, providing expert assessments, treatments, and patient education in a fast-paced hospital environment and also at Skilled nursing facility. Your expertise will help ensure seamless care transitions, optimize patient recovery, and uphold the highest standards of medical excellence.
Responsibilities
- Conduct thorough physical examinations and patient assessments to determine diagnosis and treatment plans.
- Manage acute medical care cases in General medical floors and Skilled Nursing facilities.
- Collaborate with physicians, nurses, therapists, and case managers to coordinate comprehensive patient care and discharge planning and attend daily Multidisciplinary rounds.
- Utilize Epic and Point Click Care to document medical records accurately and efficiently.
- Ensure compliance with HIPAA regulations and while maintaining high-quality patient care standards.
Experience
- Proven experience working in hospital medicine and or Skilled Nursing Facility preferred.
- Demonstrated proficiency with EMR/EHR systems such as Epic/ PointClick care.
- Strong background in managing Hospital medicine patients and Skilled Nursing facilities.
Pay: $116,176.37 – $139,911.31 per year
