Job Overview
WELS Care LLC is seeking a skilled, compassionate, and detail-oriented Nurse Practitioner (NP) or Physician Assistant (PA) to join our dedicated healthcare team as a Wound Care Specialist. In this role, you will provide specialized assessment and treatment for patients with acute and chronic wounds, playing an integral part in promoting healing, preventing complications, and improving quality of life. We are currently hiring for both a full-time provider and a contract/PRN provider.
Key Responsibilities:
- Conduct comprehensive patient assessments to evaluate the type, severity, and underlying factors contributing to wounds
- Develop, implement, and continuously refine individualized wound care plans based on clinical findings
- Perform wound cleansing, debridement, and dressing applications using evidence-based techniques and appropriate materials
- Monitor and document wound healing progress, adjusting treatment plans as clinically indicated
- Order and interpret laboratory studies, radiological imaging, and other diagnostic tests per established clinical criteria; coordinate scheduling of specialized testing as needed
- Educate patients and their families on wound care techniques, prevention strategies, and self-management best practices
- Collaborate effectively with interdisciplinary team members to ensure coordinated, patient-centered care
- Maintain accurate, thorough, and timely patient records in compliance with regulatory and organizational standards
- Demonstrate professionalism and empathy when providing care in both home settings and clinic environments
Qualifications & Skills:
The ideal candidate will possess the following:
- Current licensure as a Nurse Practitioner or Physician Assistant in the state of Utah
- Proficiency in wound assessment, management, and evidence-based treatment protocols
- Basic Life Support (BLS) certification (required)
- Ability to perform phlebotomy for laboratory testing as needed
- Strong case management skills with experience coordinating care across multiple disciplines
- Excellent clinical assessment skills with the ability to recognize and respond to changes in patient condition
- Experience in hospice or palliative medicine is a plus
- A positive, team-oriented attitude with a commitment to continuous learning and professional development

Pay: $100,000.00 – $140,000.00 per year
