Nurse Practitioner or Physician Assistant – Patient Rounding – $150,000-$160,000+

Location: Rounding in 4 SNFs near Springfield, IL – Flexible Schedule, Full Time

Job Type: Full-time, W2 Employed with benefits.

We are seeking a Nurse Practitioner or PA to join our mission-driven team. In this role, you will provide focused care in nursing facilities, helping to reduce readmissions and improve recovery for patients during their most vulnerable moments, right after a hospital stay. You will be the on-site clinical expert, making a tangible difference in the lives of geriatric patients and those undergoing rehabilitation.

We deliver physical medicine and rehab-led clinical services within skilled nursing facilities, partnering closely with therapy teams, primary care physicians, and facility staff. Our innovative model includes not only your on-site expert care but also a robust 90-day post-discharge follow-up program managed by our care coordination team, ensuring patients have the support they need to safely transition home and avoid rehospitalization.

This position involves a consistent but flexible weekday daytime schedule, typically Monday through Friday, with the flexibility to manage your own hours as long as patient needs are met. Key Responsibilities

  • Round in assigned skilled nursing facilities on a consistent weekday daytime schedule, typically spending 4 hours per day on-site.
  • Oversee daily patient interactions through timely micro-visits that support focused assessments to evaluate recovery, manage pain, treat wounds, and address functional decline.
  • Collaborate closely with physical, occupational, and speech therapists, as well as primary care physicians and facility staff, to develop and adjust individualized recovery plans.
  • Monitor readmission risks, help close gaps in post-acute care, and participate in weekly Medicare meetings to discuss patient progress and barriers to discharge.
  • Utilize EHR system (Doc Now, integrated with PointClickCare) to complete accurate and timely documentation within 48-72 hours, often remotely.

Qualifications

  • Master’s degree in Nursing and current certification as a Nurse Practitioner (NP) or Physician Assistant (PA-C) from an accredited program.
  • Active, unrestricted NP or PA license in the state of Illinois, with prescriptive authority.
  • Minimum of 2-3 years of experience in geriatrics, post-acute care, or a related field; experience in nursing facilities is strongly preferred.
  • Strong clinical skills in assessing and managing barriers to recovery, including rehabilitation, pain, and chronic conditions.
  • Excellent communication and time-management abilities to handle brief, efficient patient encounters.
  • Proficiency with EHR systems and remote documentation.
  • Ability to work independently with a high level of autonomy and consistency.

Schedule

  • Full-Time, flexible 40 hours per week.
