Founded as Boulder Hospice in 1976, TRU Community Care is a nonprofit healthcare organization serving Boulder, Denver, and surrounding counties. With a mission to affirm life at every step of the journey with illness and loss, TRU provides a continuum of care through TRU Hospice, TRU PACE (Program of All-Inclusive Care for the Elderly), TRU Palliative Care, TRU Grief Services, and Landmark Memory Care.

Position: Nurse Practitioner
Location: Lafayette,
Pay: $120,087 to #144,988 per year

JOB SUMMARY

The Nurse Practitioner works in collaboration with physicians and interdisciplinary teams as an advanced practice nurse to deliver palliative care and other health services to patients. The Nurse Practitioner will work in the community making visits at patient homes, nursing homes, and assisted living facilities, our inpatient Care Center, and will participate in conducting palliative care consults in the home and hospital settings.

EDUCATION AND QUALIFICATIONS

  • Master’s Degree.
  • Board Certified Advanced Practice Nurse in the state of Colorado.
  • Registered Nurse in the state of Colorado.
  • Controlled substance license with current DEA license number.
  • Eligible or currently possess a Medicare/Medicaid UPIN number.
  • Eligible or currently credentialed at local hospitals.
  • Eligible to bill insurance for palliative care and symptom management visits.
  • CPR Certification.
  • One or more years of working experiences as an NP. One or more years working experience as an RN preferred.
  • Certification in hospice and palliative care preferred.
  • Knowledge of Medicare, Medicaid and hospice standards of practice.
  • Excellent interpersonal communication skills.
  • Must successfully pass agency required background checks and drug test.
  • Must have $1M/$3M professional liability insurance and TRU will reimburse the premium costs
  • Must possess a valid driver’s license, provide proof of insurance (a copy of your “Declarations Page” indicating you have $100k/$300k Bodily Injury/$100k Property Damage coverage), and have reliable transportation.
  • Must comply with agency health screening requirements.
  • Must be able to demonstrate computer competency.

ESSENTIAL DUTIES

  • Works with the medical director as requested in providing palliative consult services in the community to pre-hospice patients.
  • Provides diagnosis, treatment and management services for hospice patients in the inpatient unit as well as in the home as directed.
  • Participates actively in interdisciplinary team conferences, helping to develop and revise patient care plans and acting as a medical resource in determining approaches to individual patient care.
  • Assures appropriate utilization of palliative hospice treatments based on time-limited plans, i.e., when intent is to treat symptoms.
  • Responsive to provide urgent visits (e.g. hospice readmission face-to-face encounter visits, etc.) when required during work day and when on call during the day or on the weekend.
  • Works in a safety conscious manner which insures that safe work practices are used in order not to pose a risk to self or others.
  • Utilizes standard precautions in clinical practice.
  • Understands and assures compliance with regulations and standards pertinent to state and federal requirements and Medicare/Medicaid hospice benefits.
  • Attends medical services staff meetings; meets monthly with Medical Director.
