Our providers deliver primary medical care across hospitals, skilled nursing facilities, assisted living communities, rehabilitation centers, and independent living facilities.
As our patient base continues to grow throughout the Bradenton and Sarasota areas, we are seeking an experienced and compassionate Medical Provider to join our practice in an Independent Contractor role. APRN Responsibilities:
- Travel to assigned facilities within a coordinated service area to provide care for geriatric patients
- Treat patients in nursing homes, assisted living facilities, rehabilitation centers, and independent living communities
- Perform physical examinations and routine medical evaluations
- Develop and manage individualized treatment plans
- Prepare, maintain, and update patient records using our EHR/EMR system
- Prescribe and refill medications as appropriate
- Provide referrals to specialists when necessary
- Establish strong rapport with patients and encourage open, honest communication
- Counsel patients on healthy habits and preventive care based on their current health status
- Collaborate with facility staff, family members, and other healthcare providers to ensure continuity of care
- Assist the practice physician as needed
Qualifications & Requirements
- Active, unrestricted APRN license required
- Autonomous APRN certification required
- Active DEA license required
- Experience in geriatric care preferred
- Familiarity with EHR/EMR systems
- Comfortable traveling between multiple facilities
- Ability to work independently while also collaborating effectively with physicians, staff, and facility teams
- Highly organized with strong documentation and record-keeping skills
- Strong communication skills with patients, families, providers, and staff
- Professional, respectful, and positive attitude
- Strong understanding of HIPAA compliance, patient confidentiality, healthcare practices, insurance, and billing
- Must live locally
Position Details
Job Type: Full-TimeCompensation: $120,000 annually

- Fully flexible 40-hour work week
- Call rotation: one week per month
