Job Title: Physician Assistant (PA) – Rounding Provider | Work-Life Balance + Purpose-Driven Care.
Company: Twin Cities Physicians.
Compensation: $105,000 to $120,000 annually + autonomy + career growth.

Schedule: Full-Time | Flexible, Field-Based Rounding.

Position Summary:
Responsibilities:
- Provide on-site rounding care in assisted living, memory care, and long-term care communities.
- Perform comprehensive physical exams, chronic disease management, and preventative care.
- Develop and adjust individualized care plans to improve patient outcomes.
- Collaborate with physicians, nurses, and interdisciplinary care teams.
- Educate families, caregivers, and facility staff on care coordination and treatment plans.
- Reduce ER visits and hospitalizations through proactive, consistent care.
- Complete accurate documentation, medication management, and lab coordination.
Qualifications:
- Active Minnesota-licensed Physician Assistant (PA-C) in good standing.
- Experience in geriatrics, family medicine, home health, or post-acute care preferred.
- Strong skills in clinical decision-making, documentation, and patient communication.
- Valid driver’s license and reliable transportation for mobile rounding.
- Passion for patient-centered, relationship-based senior care.

- Army: 65D – Physician Assistant.

- Air Force: 42G3 – Physician Assistant.
- Marines: Physician Assistant (230X).
- Coast Guard: Health Services Officer (HSO) / Physician Assistant.
