Company: Twin Cities Physicians.

Compensation: $105,000 to $120,000 annually + autonomy + career growth.

Location: Greater Twin Cities Metro Area.

Schedule: Full-Time | Flexible, Field-Based Rounding.

Position Summary:

This is a career-elevating opportunity for a Physician Assistant (PA-C) seeking more than the traditional clinic model. As a rounding provider, you will deliver on-site primary care, build lasting patient relationships, and operate with a high level of autonomy. Responsibilities:

  • Provide on-site rounding care in assisted living, memory care, and long-term care communities.
  • Perform comprehensive physical exams, chronic disease management, and preventative care.
  • Develop and adjust individualized care plans to improve patient outcomes.
  • Collaborate with physicians, nurses, and interdisciplinary care teams.
  • Educate families, caregivers, and facility staff on care coordination and treatment plans.
  • Reduce ER visits and hospitalizations through proactive, consistent care.
  • Complete accurate documentation, medication management, and lab coordination.

Qualifications:

  • Active Minnesota-licensed Physician Assistant (PA-C) in good standing.
  • Experience in geriatrics, family medicine, home health, or post-acute care preferred.
  • Strong skills in clinical decision-making, documentation, and patient communication.
  • Valid driver’s license and reliable transportation for mobile rounding.
  • Passion for patient-centered, relationship-based senior care.

Military Occupational Classification (MOC) Codes:

  • Army: 65D – Physician Assistant.
  • Navy: Physician Assistant (230X).
  • Air Force: 42G3 – Physician Assistant.
  • Marines: Physician Assistant (230X).
  • Coast Guard: Health Services Officer (HSO) / Physician Assistant.
