Job Description:We are seeking a skilled and compassionate PA specializing in Advanced Primary Care and Risk Management. This role focuses on delivering high-touch, relationship-based care supported by innovative technology and actionable data. As an integral part of our advanced care team, you will focus on managing patient risk, promoting wellness, and providing preventive care through a personalized approach that addresses patients’ individual needs.

Key Responsibilities:

  • Deliver high-quality, evidence-based primary care to patients, focusing on risk reduction and proactive management of chronic and acute conditions.
  • Develop personalized, data-informed care plans that leverage technology and actionable data to provide insights on patient risk factors and health trends.
  • Utilize telemedicine, digital health tools, and data analytics to monitor, assess, and adjust patient care plans in real-time.
  • Collaborate with a multidisciplinary team, including physicians, care managers, and data analysts, to enhance patient outcomes through coordinated and comprehensive care.
  • Conduct in-depth patient assessments, interpret clinical data, and adjust treatment plans based on emerging data insights and patient progress.
  • Educate patients and their families on preventative health strategies, lifestyle modifications, and the effective management of chronic conditions.
  • Participate in quality improvement initiatives and contribute to the continuous enhancement of our advanced primary care model.

Qualifications:

  • Current and valid Physician Assistant (PA) license in Indiana or eligible for it.
  • Master’s or Doctorate degree in Nursing with board certification in Family Medicine, Internal Medicine, or a related specialty.
  • At least 3 years of experience in primary care, chronic disease management, or a related field.
  • Strong understanding of risk management principles, advanced primary care models, and patient-centered healthcare.
  • Experience with healthcare technology, including electronic health records (EHR), telemedicine platforms, and data analytics tools.
  • Excellent communication skills, with a focus on patient education, collaborative care, and problem-solving.
