Unique home visiting based opportunity to service clients in the community. APP will travel between 6-30 miles daily to visit clients.
- Conducting thorough physical examinations.
- Obtaining detailed medical, social, and family histories.
- Evaluating patients’ living conditions and social determinants of health
- Diagnosing acute and chronic conditions.
- Developing and implementing individualized care plans.
- Prescribing medications and treatments as needed.
- Collaborating with other healthcare professionals, such as physicians, specialists, and social workers.
- Coordinating referrals to other healthcare services or community resources.
- Ensuring continuity of care through regular follow-ups and monitoring.
- Educating patients and their families about disease management, medications, and lifestyle modifications.
- Guiding preventive care and health maintenance.
- Managing urgent health issues that arise during home visits.

- Maintaining accurate and up-to-date patient records.
- Documenting all assessments, treatments, and patient interactions.
- Reporting any changes in patient condition to the appropriate healthcare providers
- Advocating for patients’ needs and preferences.
- Offering emotional support to patients and their families.
- Assisting with navigating healthcare systems and accessing necessary services.
- Participating in quality improvement initiatives to enhance patient care.

GENERAL SUMMARY: Provides inpatient and/or outpatient medical care. Takes patient history, performs physical examination, orders, and interprets tests, makes diagnoses, and institutes appropriate treatment programs. Works in collaboration with staff physicians, house officers, nurses, and other medical care team members to ensure that patient outcomes are achieved safely and efficiently. Specific procedures are individually credentialed as indicated by training and experience.
EDUCATION/EXPERIENCE REQUIRED:
- Master’s degree in nursing -OR- requires a bachelor’s degree, completion of an AMA-approved Physician Assistant program, Licensure as a Physician Assistant
- 1 year APP experience required
CERTIFICATIONS/LICENSURES REQUIRED:
- Certified Nurse Practitioner. Licensed as a Registered Nurse or Licensure as a Physician Assistant
- Current BLS or ACLS as required.
- Current ATLS/PALS provider status as required by department.
