MHS Mobile Docs is a premier group of compassionate medical providers committed to establishing comprehensive and consistent healthcare for homebound and elderly patients. We ensure prompt evaluations to address patients’ needs through proactive, quality healthcare in the comfort of their home and skilled nursing facilities.
Position Summary:
The APP goes to patients’ homes and SNFs to develop an individualized plan of care for each patients. The following responsibilities include, but are not limited to:
- KEEP PROMPT COMMUNICATION WITH OFFICE STAFF (CONFIRMING APPOINTMENTS, SENDING IN NEEDED FORMS)
- PROVIDING ADMISSION FORMS AND PROGRESS NOTES WITHIN 24HRS OF INITIAL PATIENT VISIT (ALL INFORMATION PROPERLY FILLED OUT)
- REVIEWING PATIENT INFORMATION AS NEEDED (H&P, MED. LIST, ETC.)
- COMMUNICATING PROFESSIONALLY WITH PATIENT AND/OR PATIENT FAMILY BEFORE AND THROUGHOUT PATIENT VISITS
Requirements:
- Must Possess an active, unrestricted CA License
- Reliable Transportation
- Current BLS/ACLS
- Part-time/per diem, Flexible schedule, 1099
- Experience preferred, but will consider motivated new grad
