We are seeking a qualified and experienced Physician Assistant/ Nurse Practitioner for a full-time position in a primary care setting. The role involves providing comprehensive care for adult patients, with a particular emphasis on diabetes management and weight loss.

Responsibilities:

• Perform patient assessments, including medical history, physical exams, and diagnostic evaluations.
• Manage chronic conditions, particularly diabetes, and guide patients in weight loss and lifestyle modifications.
• Develop and implement treatment plans, including prescribing medications and managing follow-up care.
• Educate patients on disease management, prevention, and health maintenance.
• Collaborate with a multidisciplinary team to ensure the best outcomes for patients.
• Maintain accurate and up-to-date patient records in compliance with healthcare regulations.
• Provide care during weekdays only, with no weekend work required.

Qualifications:

• Current and valid PA or NP license in California.
• Minimum of 2 years of experience in primary care, with a focus on adult patients.
• Strong expertise in diabetes management and weight loss interventions.
• Excellent interpersonal and communication skills.
• Ability to work independently and collaboratively within a healthcare team.
• Fluent in Spanish is required.
