Senior Doc is currently hiring Nurse Practitioners (NP) / Physician Assistants (PA) with the passion and experience of serving patients residing in Senior Living communities. Position Responsibilities:
Positional responsibilities may include, but are not limited to:
- Perform physical exams, diagnostic test interpretation, and medication reconciliation.
- Provide primary care to elderly patients and update their families the plan of care.
- Patient education on health and wellness topics which includes lifestyle changes to improve their health.
- Collaborate with other healthcare professionals to coordinate patient care.
- Cultivate and strengthen relationships with partnered facilities through education and engagement in community events.
- On-site in senior living communities, in assigned service area.
Candidate Qualifications:
Interested candidates must have knowledge, experience, and confidence in managing complex chronic and acute medical conditions in ALFs and SNFs.
- Minimum one year experience in Adult/Geriatric care preferred
- Current NP/ PA license in good standing
- Comfortable with end of life discussions
- Experience with virtual care/telehealth
- Excellent attention to detail
- Adaptable and open to change and problem-solving.
- Proficient written and verbal communication skills
- Ability to independently diagnose, prescribe and treat common chronic medical conditions
- Highly motivated
- Resilience
- Team player
- Strong interpersonal skills to develop and cultivate relationships with healthcare team and patients
- Knowledge of Medicare Advantage Managed Care and CMS guidelines
- Knowledge of HCC, ICD-10 coding, and documentation
- Experience and knowledge of and ability to maintain HIPAA requirements
- Ability to stoop, kneel and crouch required.
- Ability to bend, reach, lift, pull, push, and carry up to 10 lbs. regularly, and up to 50 lbs. occasionally

- The posted salary reflects the starting range of total compensation and may include productivity bonuses, incentives related to quality and performance, on-call stipends, extra shift incentives, and other forms of cash compensation as applicable to the position. In addition, providers typically receive a CME allowance and other benefits offered by their medical group employer. Please note that this salary range is provided in accordance with State law and is subject to variation due to the factors noted above.

Pay: $105,000.00 – $187,500.00 per year
