Nurse Practitioner Job Description
Job SummaryWe are seeking a dedicated and compassionate Nurse Practitioner to join our healthcare team. The ideal candidate will have a strong background in geriatrics and experience in memory care, with the ability to provide high-quality patient care in various settings, including clinics and telehealth environments. This role requires proficiency in medical terminology and familiarity with electronic health record systems such as Cerner. The Nurse Practitioner will be responsible for assessing patient needs, developing treatment plans, and collaborating with other healthcare professionals to ensure optimal patient outcomes.
Responsibilities
- Conduct comprehensive assessments of patients, including physical examinations and health histories.
- Develop and implement individualized care plans based on patient needs and preferences.
- Monitor patient progress and adjust treatment plans as necessary.
- Provide education to patients and their families regarding health management, medication adherence, and disease prevention.
- Collaborate with interdisciplinary teams to coordinate care for patients, particularly those requiring tube feeding or specialized interventions.
- Uphold infection control protocols to ensure a safe environment for patients and staff.
- Maintain accurate documentation of patient encounters in compliance with legal and regulatory standards.
- Utilize telehealth technologies to provide remote consultations and follow-up care when appropriate.
Schedule: 12-hour day shifts, 7 on/7 off
No call required
Daily Volume: 3-5 surgeries, 15 patients, 5 admits
Duties: Pre-op, post-op, first assist
Team Structure: 7 MDs, 6 APPs
EMR: Epic
Must be board-certified

Experience
- A Master’s degree in Nursing from an accredited program is required.
- Current Nurse Practitioner certification and state licensure are mandatory.
- Experience in geriatrics or memory care is highly preferred.
- Familiarity with electronic health record systems, particularly Cerner, is advantageous.
- Knowledge of medical terminology relevant to clinical practice is essential.
- Previous experience in infection control practices is beneficial.
- Exposure to sonography techniques may be considered a plus.

Pay: $131,907.00 – $144,037.00 per year
