Full Job Description

At Rejuvenation Medical, we work as a team to provide care to patients in post acute facilities (primarily nursing homes) as well as at our primary care clinic to provide uniquely personalized patient-centric care while integrating with the team at the buildings we serve. No other post acute team comes close to our level of accommodating patients’ and facilities’ needs. We are proud of what we do.

Primary Responsibilities:

  • Conduct physical assessments
  • Manage patients through an acute medical situation or an exacerbation of a chronic condition, including testing, diagnostics, interventions
  • Manage members during a short term sub-acute rehabilitation episode
  • Ensure accurate and complete documentation, including ICD 10 conditions
  • Communicate and collaborate with the interdisciplinary care team
  • Conduct advanced illness and advanced care planning conversations
  • Provide patients and caregivers with counseling and education
  • Adherence to medical policies, protocols, criteria, and clinical guidelines to ensure best practices are maintained for clinically effective and efficient care delivery.
  • Adherence to compliance policies, procedures, and standards of conduct including all applicable laws and regulations.
  • Supportive function as growth/engagement ambassador in initiating, developing, and sustaining relationships with patients, caregivers, congregate living facility staff, and clinical partners.
  • Participation in after-hours on-call schedule and other clinical care cross-coverage as requested.
  • Attend organizational meetings bimonthly

Required Qualifications:

  • Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice or Physician’s Assistant program
  • Active and unrestricted license in Washington state, or ability to obtain by start date
  • Active Nurse Practitioner/Physician’s Assistant certification through a national board:
  • Board certified through the American Academy of Nurse Practitioners or the American Nurses Credentialing Center, with certification in one of the following:
  • Family Nurse Practitioner
  • Adult Nurse Practitioner
  • Gerontology Nurse Practitioner
  • Adult-Gerontology Acute Care Nurse Practitioner
  • Physician Assistants
  • Current, active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations (unless prohibited in state of practice)
  • Ability to complete physical requirements of the job including lifting a 30-pound bag in and out of the car, navigating stairs and a variety of dwelling conditions, and sit, stand, and kneel to perform physical assessment
  • Access to reliable transportation
  • Ability to interact and communicate with all stakeholders in a compassionate and patient centric way

Preferred Qualifications:

  • 1+ years of clinical experience in practice (long-term care setting preferred)
  • Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs
  • Understanding of Geriatrics and Chronic Illness
  • Understanding of Advanced Illness and end of life discussions
  • Proficient computer skills including the ability to document medical information with written and electronic medical records

Job Types: Full-time, Part-time

Pay: $120,000.00 – $143,000.00 per year

Expected hours: 32 – 42 per week
