The Mission: Closing the Gap You will help build a clinical bridge for individuals transitioning from city and county jails back into the community. You will provide: Medication management for high-acuity patients Forensic diversion support Integrated physical + psychiatric triage Continuity of care during critical transitions This work directly supports some of the most vulnerable individuals in the system, ensuring stability where gaps often exist.
