Full Job Description
BioMend Health Solutions, PLLC is a mobile primary care and wound care practice providing in-home, community-based medical services to older adult and geriatric patients throughout Washington State.
Our providers deliver care directly to patients wherever they reside, including private homes, skilled nursing facilities, assisted living communities, and other residential or care settings. As a statewide mobile provider, we improve access, continuity, and outcomes for individuals with complex and chronic health needs across urban, suburban, and rural communities.
We are seeking Family Nurse Practitioners (FNPs) for per diem, as-needed coverage to provide mobile primary care and wound management, and Physician Assistants (PAs) to provide wound care services, based on experience, scope of practice, and credentialing.

Requirements:
- Provide mobile primary care services to older adults and geriatric patients (Family Nurse Practitioner)
- Provide wound care services, including advanced wound management and skin substitute procedures(Family Nurse Practitioners and Physician Assistants)
- Conduct comprehensive assessments and develop individualized treatment plans within the scope of practice
- Manage chronic and acute conditions commonly seen in aging populations (FNP role)
- Utilize the organization’s electronic health record (EHR) for accurate, timely, and compliant clinical documentation
- Coordinate care with interdisciplinary teams and facility staff as appropriate
- Follow established clinical protocols, documentation standards, and quality expectations
- Travel within Washington State as needed based on patient location
