Full Job Description

Key Responsibilities:

  • Patient Care:
  • Conduct thorough patient assessments, including medical histories, physical exams, and diagnostic evaluations.
  • Develop and implement individualized care plans for patients with chronic, acute, and non-healing wounds.
  • Perform procedures related to wound care, including debridement, dressing changes, wound measurements, and other interventions as necessary.
  • Educate patients on proper wound care, preventive measures, and lifestyle changes to support wound healing.
  • Monitor patient progress, adjusting treatment plans as needed and providing follow-up care.
  • Collaborate with physicians and other healthcare providers to ensure comprehensive patient care.
  • Clinical Documentation:
  • Maintain accurate, timely, and detailed patient records in accordance with medical and regulatory standards.
  • Document wound assessments, treatment plans, and patient progress in electronic health records (EHR).
  • Collaboration and Communication:
  • Work closely with the medical team to discuss patient treatment plans and outcomes.
  • Communicate effectively with patients, their families, and healthcare team members to ensure cohesive care.
  • Participate in team meetings, case reviews, and educational sessions to enhance knowledge and provide the best care.
  • Patient Education & Support:
  • Educate patients on wound prevention, proper nutrition, and lifestyle adjustments to promote faster healing.
  • Provide emotional support and counseling to patients dealing with chronic or severe wounds.
  • Continuous Improvement & Learning:
  • Stay up to date with the latest trends, research, and advancements in wound care.
  • Participate in ongoing professional development and training in wound care and related fields.

Job Type: Full-time

Pay: $80,000.00 – $90,000.00 per year

License/Certification:

  • Certified Nurse Practitioner (Preferred)

Ability to Relocate:

  • Newport News, VA 23608: Relocate before starting work (Required)
