OverviewWe are seeking a dedicated and compassionate advanced practice clinician specializing in nursing home and assisted living care to join our dynamic healthcare team. This role involves providing comprehensive medical oversight and personalized patient care within long-term care facilities. As an NP or PA in this setting, you will be instrumental in enhancing the quality of life for our residents by delivering expert medical assessments, managing complex health conditions, and coordinating multidisciplinary care plans. Your expertise will help foster a safe, supportive environment where residents feel valued and well-cared for.
Duties
- Conduct thorough medical evaluations and physical examinations of residents to assess their health status and identify medical needs.
- Develop, implement, and regularly update individualized care plans tailored to each resident’s unique health requirements.
- Manage acute and chronic health conditions, including medication management, treatment adjustments, and necessary interventions.
- Collaborate closely with nursing staff, specialists, therapists, and family members to coordinate comprehensive care delivery.
- Oversee medication administration processes, ensuring adherence to safety protocols and regulatory standards.
- Perform routine follow-ups, monitor vital signs, and document all clinical interactions accurately within electronic health record (EHR) systems.
- Participate in quality improvement initiatives, discharge planning, and clinical research activities to enhance resident outcomes.
Skills
- Knowledge of and willingness to learn medical terminology, physiology, and pathology relevant to geriatric populations.
- Proficiency with electronic health record (EHR) systems.
- Experience with inpatient care settings, hospital medicine, or post-acute care environments is highly desirable.
- Familiarity with specialized fields such as psychiatry, cardiology, endocrinology, nephrology, pulmonology, or neurology is advantageous.
- Ability to perform comprehensive physical examinations and diagnostic evaluations efficiently.
