About Project Reality

Established over 50 years ago, Project Reality partners with Salt Lake County and Utah State to deliver services that impact the underlying causes of chronic disease and improve community health outcomes. We are a fully integrated health clinic providing comprehensive substance use disorder, primary care, and mental health treatment for vulnerable populations not adequately served in traditional health care settings. Our health home model addresses critical health determinants through a team-based approach utilizing medical providers, therapists, case managers, and peer coaches. We are seeking a part-time or full-time Board Certified/Board Eligible Physician Assistant.

Responsibilities

  • Assess, diagnose and treat acute and chronic medical conditions for adults with complex psycho-social challenges.
  • Work closely with psychiatrist and counselors to integrate family medicine considerations into a comprehensive care plan that includes behavioral components and medication assisted treatment with methadone, buprenorphine, or long-acting naltrexone; be willing to partner with psychiatric and behavioral health specialists.
  • Advance the agency’s knowledge of chronic disease management through the development of medical protocols and educational resources for primary care services; advise the Medical Director and Executive Director on primary care needs of the population served.
  • Provide excellent patient-centered care and promote total population health beyond individual needs while being a good steward of limited health resources and public funding sources.
  • Coordinate with Project Reality partners including speciality medical clinics, OB/Gyn providers, residential treatment programs, and Salt Lake County Jail.
  • Complete accurate and timely documentation of clinical care; code and bill accurately per established protocol; ensure that records are detailed and complete, and comply with standards and regulations.
  • Attend professional conferences, ECHO sessions as needed, and participate in community activities to educate and advance care for patients with dual diagnoses and chronic medical conditions

Experience

  • Active UtahT PA License; current DEA license preferred
  • NCCPA Board Certification
  • Must have a strong commitment to a team dynamic as well as the ability to take initiative.
  • Must have good communication and interpersonal skills as well as an ability to effectively relate to all medical staff and patients.
  • Must be able to pass and maintain DHS/BCI background screening
  • Part-time or full-time status will depend on the candidates experience.
