Mobile Nurse Practitioner/Physicians Assistant
Reports to: Owner/CEO
Job Description:
Mobile Providers provide primary care, care coordination, oversight, and health and wellness related care to a variety of patients in various ages/stages. Care is provided at the patient’s residence. Palliative care and pain management are also areas of specialty and focus. The geriatric population comprises the majority of the patient’s at this time. As Vitality Health of Utah expands, the providers will have opportunities to focus on a specialized area of practice if desired.
Vitality providers are expected to maintain a visit caseload commensurate with their desired salary. While maintaining their caseload, the Associate Mobile Provider nshould always maintain a kind, and professional demeanor with their patients, family members of patients, and the community and facility staff members.

Essential Job Duties:
- Conduct patient visits in a kind, and orderly manner.
- Keep up knowledge of industry standards for treatment and care related to primary patients focus.
- Complete thorough assessments and develop plans of care in accordance with appropriate SOPs.
- Maintain or develop a strong desire for personal development, growing clinically and professionally through reading, discussion, and other forms of education.
- Have positive and uplifting interactions everywhere you go. We don’t ask that you project false positivity, but we should strive to leave each interaction having left someone slightly better than before the interaction.
Job Qualifications:
- Must have completed their Masters or Doctor of Nursing and have an unencumbered Utah Nurse Practitioner License OR;
- Must have completed their Masters of Physician assistant Studies (MPAS) or a Master of Science in Physician Assistant Studies (MSPAS) and have passed the PANCE as well as have an unencumbered Physician Assistant license in the state of Utah.
- 1-2 years of clinical experience in a mobile setting such as Home Health, Hospice, or Mobile care, or an understanding of this model of care delivery, is preferred.
Universal Precautions:
Exposure to blood/bodily fluids is unlikely.
Physical Requirements:
Can do occasional light to moderate lifting of supplies and help patients move or adjust (up to over 40 lbs.).
Job Type: Full-time
Pay: $95,000.00 – $140,000.00 per year
