Job Summary:
We are seeking a dedicated and experienced Nurse Practitioner (NP) to provide high-quality, patient-centered care in a post-acute setting. The NP will work collaboratively with physicians, nursing staff, and interdisciplinary teams to manage the medical needs of patients transitioning from hospital to skilled nursing, rehabilitation, or long-term care facilities. This role focuses on improving patient outcomes, reducing hospital readmissions, and ensuring continuity of care.
Key Responsibilities:
- Perform comprehensive assessments, including history and physical exams for post-acute patients
- Develop, implement, and manage individualized care plans
- Diagnose and treat acute and chronic conditions
- Order, interpret, and evaluate diagnostic tests and labs
- Collaborate with physicians, nurses, therapists, and facility staff to coordinate care
- Provide timely follow-ups and manage transitions of care
- Educate patients and families on treatment plans, medications, and preventive care
- Ensure accurate and timely documentation in compliance with regulatory standards
- Participate in quality improvement initiatives aimed at reducing readmissions and improving outcomes
Qualifications:
- Current, active Nurse Practitioner license in Texas
- Board certification as a Nurse Practitioner (e.g., FNP, AGNP)
- DEA license and prescriptive authority
- Minimum [1–2+] years of experience in post-acute care, geriatrics, internal medicine, or related field preferred
- Strong clinical decision-making and diagnostic skills
- Excellent communication and interpersonal skills
- Ability to work independently and collaboratively in a fast-paced environment
Preferred Skills:
- Experience in skilled nursing facilities (SNF), long-term care, or rehabilitation settings
- Familiarity with EMR systems
- Knowledge of CMS guidelines and post-acute care regulations

Pay: $35.00 – $40.00 per hour
