Nurse Practitioner / Physician Assistant – Internal MedicineJob Summary
We are seeking a bilingual English/Spanish Nurse Practitioner or Physician Assistant to join our growing Internal Medicine practice.
This role focuses on adult and geriatric primary care, with an emphasis on chronic disease management, Medicare Advantage, preventive care, and long-term patient relationships.
Responsibilities
- Evaluate, diagnose, and treat adult and geriatric patients
- Manage chronic conditions including diabetes, hypertension, hyperlipidemia, COPD, CKD, obesity, and cardiovascular disease
- Perform Medicare Annual Wellness Visits and routine preventive care
- Identify and close quality and preventive care gaps
- Manage acute concerns in the outpatient setting
- Review labs, imaging, hospital records, and specialist recommendations
- Coordinate care with physicians, specialists, and care management teams
- Provide accurate and timely medical documentation
- Support Medicare Advantage and value-based care initiatives
Requirements
- Current Texas NP or PA license
- National certification
- Fluent Spanish required
- Strong clinical and communication skills
- Experience in Internal Medicine, Family Medicine, Primary Care, or Geriatrics preferred
- Medicare Advantage or value-based care experience preferred
- Knowledge of ICD-10 and chronic disease documentation preferred
- Athenahealth EMR experience is a plus
- Ability to work effectively in a collaborative, fast-paced outpatient practice
We are looking for a compassionate, dependable provider who enjoys caring for patients over time and wants to grow with a high-quality Internal Medicine practice.
Pay: $120,000.00 – $160,000.00 per year
