Psychiatric Mental Health Nurse Practitioner (PMHNP-BC) — Outpatient Practice | Harlingen, TX

Job Type: Full-time, W2

Schedule: 4 days/week, 9:00 AM – 6:00 PM

Location: Harlingen, TX (on-site outpatient)

Compensation: $120,000–$135,000 base + performance incentives

About Transitions Behavioral Health

Transitions Behavioral Health is a growing psychiatric practice serving the Rio Grande Valley. Founded by Velma Henn, PMHNP-BC, we provide outpatient psychiatric care across four clinical pillars: women’s mental health and perimenopause, anxiety and depression, ADHD and life transitions, and adolescents and young adults.

We’re not a volume mill. We’re building a practice where providers have time to think, space to practice well, and systems that remove administrative burden from clinical care. Our patient demand is growing faster than our capacity — we’re hiring because our panels are full, not because we’re trying to fill chairs.

What You’ll Do

Provide outpatient psychiatric evaluations, medication management, and brief psychotherapy to adolescents and adults

Diagnose and document with ICD-10 specificity to support clinical outcomes and payer compliance

Work within a multidisciplinary team of PMHNPs and an LCSW, with internal therapy referrals readily available

Document efficiently with AI-assisted charting tools (ambient scribe rolling out Summer 2026) and athenaOne EHR

Collaborate under an established Collaborative Practice Agreement with a supervising psychiatrist

Who You Are

PMHNP-BC (board-certified)

Active Texas APRN license with prescriptive authority and DEA

Credentialed with Medicare (preferred) and/or commercial panels (BCBS, Aetna, Cigna, UHC)

Bilingual English/Spanish strongly preferred

1+ year of independent psychiatric practice experience

Familiarity with trauma-informed care principles

Committed to evidence-based, compassionate care
