We are seeking a highly qualified Nurse Practitioner (NP) or Physician Assistant (PA) to support our Skilled Nursing Facility (SNF) partners and their patient populations. This role is primarily focused on delivering high-quality post-acute and long-term care within SNF settings.

This position offers flexibility in scheduling (full-time or part-time depending on candidate availability) and opportunities for growth within a rapidly expanding physician-led organization.

This role may serve patients in [County] County. Ideal candidates will reside in or near the [Neighboring City] area or be able to reliably commute to assigned facilities.

Compensation

  • $18 per RVU
  • Approximately 2.7 RVUs per patient encounter
  • Effective rate: approximately $48.60 per patient encounter
  • Production-based model with strong earning potential based on patient volume and efficiency

Candidate QualificationsRequired

  • Certified Nurse Practitioner (NP) or Physician Assistant (PA)
  • Licensed or eligible for unrestricted licensure in the state
  • Ability to work independently in skilled nursing facility settings
  • Strong clinical judgment and professionalism

Preferred

  • Minimum 1 year of post-acute care, SNF, or long-term care experience
  • Experience managing geriatric or medically complex patients
  • Strong documentation and EMR proficiency

Candidate Responsibilities

  • Provide comprehensive medical care to patients in Skilled Nursing Facilities (SNFs)
  • Perform clinical assessments and manage acute and chronic conditions
  • Collaborate with physicians, nursing staff, facility leadership, and interdisciplinary teams
  • Order and interpret diagnostic tests as appropriate
  • Perform procedures within scope and credentialing
  • Complete accurate and timely documentation, including progress notes, admission, and transfer orders
  • Ensure compliance with SNF regulations and quality standards
  • Educate patients and families on disease processes and treatment plans
  • Support continuity of care across transitions within the post-acute setting
