Job Overview
We are seeking a highly skilled and compassionate Family Nurse Practitioner (FNP) to join our healthcare team. The ideal candidate will possess extensive clinical expertise across a broad spectrum of medical disciplines, including primary care, urgent care, geriatrics, pediatrics, and specialized areas such as infectious disease care, behavioral health, and post-acute care. The FNP will provide comprehensive patient assessments, develop treatment plans, and deliver high-quality medical services in diverse settings such as outpatient clinics, hospitals, and telehealth platforms. Experience with EMR/EHR systems like Glace, and eClinicalWorks is essential to ensure seamless documentation and patient management. This role offers an opportunity to work with a multidisciplinary team dedicated to delivering exceptional patient-centered care while utilizing advanced clinical skills.
Duties
- Conduct thorough physical examinations and patient assessments across all age groups, including pediatrics, geriatrics, neonatal, and adult populations.
- Manage acute and chronic conditions such as pain management, infectious diseases, behavioral health issues, and post-surgical recovery.
- Perform diagnostic evaluations utilizing laboratory testing, medical imaging, sonography, and other advanced diagnostic tools.
- Administer medications including injections, IV infusions, while adhering to sterile techniques.
- Develop and implement individualized care plans incorporating discharge planning, nutrition counseling, health coaching, family planning, and memory or dementia care strategies.
- Utilize EMR/EHR systems like Glace or eClinicals Works for accurate medical documentation including CPT coding and ICD coding (ICD-10).
- Engage in telehealth consultations to expand access to care while maintaining compliance with HIPAA regulations.
- Participate in case management activities including utilization review and outpatient/inpatient coordination.
- Maintain infection control standards and aseptic techniques during all clinical procedures.
- Stay current with advancements in medical terminology, physiology knowledge, and emerging healthcare practices to ensure best practices.
Skills
- Proven experience with EMR/EHR systems such as Glace, Cerner (eClinicalWorks), or Athenahealth for efficient documentation.
- Expertise in diverse clinical procedures including suturing, IV insertion/infusion, phlebotomy/venipuncture.
- Strong knowledge of pharmacology for medication administration including injections and dermal fillers.
- Ability to perform diagnostic evaluations using laboratory experience or medical imaging techniques like sonography or laboratory specimen processing.
- Skilled in managing complex cases involving acute pain management, hospice & palliative medicine, occupational health issues, or infectious disease care.
- Experience working with vulnerable populations such as assisted living residents or individuals with developmental disabilities or dementia care needs.
- Excellent communication skills for patient education on family planning or nutrition; adept at working with multidisciplinary teams across outpatient clinics or inpatient units.
- Knowledge of medical coding systems (CPT codes; ICD-9/10) for accurate billing documentation.
