Full Job Description

Job OverviewWe are seeking an experienced Nurse Practitioner (NP) to join our Internal Medicine practice. The ideal candidate will be capable of working in a fast-paced clinical environment, managing a panel of adult and geriatric patients with multiple conditions. This role requires strong clinical judgment, efficiency in chronic disease management.

Duties

  • Provide comprehensive evaluation, diagnosis, and management of adult and geriatric patients with complex chronic and acute medical conditions.
  • Conduct preventive health assessments, annual wellness visits, and follow-up visits in accordance with evidence-based guidelines.
  • Collaborate with supervising physicians, medical assistants, and care coordination staff to ensure continuity and quality of care.
  • Review and reconcile medications, order diagnostic tests, interpret results, and adjust care plans as appropriate.
  • Ensure accurate and timely documentation in the EMR system consistent with billing and compliance standards.
  • Participate in quality improvement initiatives and assist in achieving organizational goals related to HEDIS measures, STAR ratings, and ACO/Value-Based Care programs.
  • Provide patient education and counseling on disease management, preventive care, and health maintenance.
  • Manage patient care independently while seeking consultation for complex or uncertain cases as needed.

Qualifications

  • Current Texas Nurse Practitioner license and Board Certification (FNP or Adult-Gerontology NP) required.
  • Minimum 2–3 years of experience in internal medicine, primary care, or a comparable healthcare setting.
  • Bilingual preferred
  • Strong clinical acumen in managing chronic diseases such as diabetes, hypertension, COPD, CHF, and multi-comorbidity cases.
  • Familiarity with HEDIS, STAR ratings,and Medicare documentation standards.
  • Excellent communication, teamwork, and patient-relationship skills.
  • Comfortable working independently and managing a full patient schedule.
  • Experience with EMR systems (Practice Fusion, Tebra, Athena, or similar) preferred.

Job Type: Full-time

Pay: $120,000.00 – $130,000.00 per year
