Full Job Description
CLINICAL DUTIES AND RESPONSIBILITIES:
- Assess health status of assigned patients through physical examination and medical history.
- Analyze patient data and formulate an individualize plan of care based on patient assessment and needs, including but not limited to pain, symptom management and psychosocial distress in collaboration with physicians, nursing, and other members of the interdisciplinary team.
- Evaluate and monitor the effectiveness of prescribed management plan and be able to pivot as needed.
- Establish priorities based on patient needs and act on these accordingly.
- Assist patient and family in understanding of illness and treatment options as related to pain management as well as defining goals of care and document in the EMR taking into consideration cultural differences.
- Utilize teaching/learning principles coupled with an expert clinical knowledge of pain management to promote and provide education and counseling to the patient and/or family including teach back.
- Act as a liaison for patients, families, and other health professionals to achieve the best quality of care for the patient experiencing pain.
- Attends, participates in discharge planning and/or multi-disciplinary rounds or family meetings when needed to promote high quality of care.
- Collaborates with Pain Management Medical Director on system-wide pain education and multi-disciplinary protocol development.
- Participate in the collection of research data, quality improvement, discharge planning and coordination of patient care for consulted patients.
EDUCATION AND EXPERIENCE REQUIREMENTS:
- Graduate of an accredited Advanced Nurse Practitioner Program or Physician Assistant program.
- Acute Care NP certification is required
- Candidate must be licensed or license eligible in the state of Texas
- Prescriptive authority required
- DEA Registration required or pending
- Two or more years’ experience as a provider preferred/negotiable
- Pain management experience desirable, but not required.
- experience with wellness and functional medicine welcome
