We’re currently expanding in the Rockwall, TX area and seeking a compassionate, skilled Nurse Practitioner (NP) or Physician Assistant (PA) to join our field-based team. Position Summary
As a Mobile Wound Care Provider, you’ll travel to patient homes, assisted living communities, and skilled nursing facilities to deliver in-person wound care services. This is an independent contractor position, compensated at $90 per patient visit. Your caseload will include both standard and advanced wounds requiring ongoing assessment and clinical decision-making.

Key Responsibilities
Wound Care Treatment
- Perform standard and advanced wound care, including:
- Skin graft application
- Debridement (sharp, enzymatic, or autolytic)
- Dressing selection and application
- Infection management
- Identify wound types and stages and adapt care accordingly
Mobile Patient Visits
- Travel to patient residences and long-term care facilities across Rockwall and surrounding areas
- Maintain a consistent, punctual visit schedule based on patient volume
Clinical Evaluation & Care Planning
- Conduct full evaluations of wound types, depth, infection status, and surrounding tissue
- Develop customized treatment plans tailored to each patient’s condition and healing trajectory
- Order appropriate products or referrals as needed
Communication & Coordination
- Collaborate with referring physicians, specialists, and home health agencies
- Provide regular updates on patient status and healing outcomes
Documentation & Reporting
- Use EHR systems to document evaluations, progress, treatments, and communication in a timely and compliant manner
Education & Support
- Teach patients and caregivers about wound prevention, signs of infection, self-care techniques, and dressing changes
- Help patients remain engaged and informed in their healing process
Requirements
Credentials & Experience
- Licensed Nurse Practitioner (NP) or Physician Assistant (PA) in the state of Texas
- Minimum 1 year of clinical experience in wound care, home health, geriatrics, or related field preferred
- Familiarity with advanced wound care techniques and products
Logistics
- Must have reliable transportation and a valid driver’s license
- Willingness to drive throughout the Rockwall area and nearby regions
- Ability to transport basic medical supplies and equipment as needed
