Job Description

Assumes responsibility and accountability for the nursing assessment and/or primary care of patients. Maintains currency of licensure and certification. As applicable serves as preceptor and clinical instructor for graduate/student/ nurses. Functions under collaborating physicians.Principal Duties and Responsibilities

  • Provides guidance, screening and health education to patients to promote and maintain health.
  • Initiates treatment for patients. Prescribes, coordinates, delegates and evaluates care.
  • Prescribes medications as a function delegated by the physician consultant(s)
  • Monitors patients with stabilized long term or chronic illness
  • Plans appropriate care utilizing community resources and acts as a liaison by relaying information necessary for continuity of care to other health professionals and clinic partners
  • Instructs individual and/or families or groups about health or defined health problems.
  • Participates in Quality Assurance activities.
  • Participates in research projects
  • Participates in education of graduate nursing students as assigned.
  • Promotes guest relations; anticipates and responds to the needs of others; provides assistance in a courteous, quick manner; treats others with care and respect maintaining privacy, confidentiality and dignity.
  • Maintains a professional appearance and a clean, safe environment.
  • Responsible for training of patients individually and in groups regarding nutritional counseling, life style changes that promote improved health outcomes and ongoing preventive health education specifically relating to chronic endemic health issues, i.e. diabetes, hypertension, TB, depression, etc.

Skills Required

  • Able to plan and organize work. (Self-starter professional, minimal supervision)
  • Culturally aware of target population.
  • Effective verbal and written communication skills.
  • Interpersonal skills necessary to work productively with patient, families, nursing staff, and other departments.
  • Knowledge of community resources.
  • Strong EMR and computer software skills.

Job Qualifications

  • Physician Assistant currently registered with the Texas State Board of Medical Examiners
  • Registered Nurse licensed in the State of Texas
  • Nurse Practitioner currently registered with the Texas State Board of Nursing Examiners (BNE)
  • Masters degree in Nursing, specialized in Family Practice

Job Type: Full-time

Pay: $60.00 – $65.00 per hour
