Full Job Description
Responsibilities:

Conduct telephonic consultations with patients to assess their weight loss goals, medical history, lifestyle factors, and dietary habits.Develop personalized weight loss plans tailored to each patient’s unique needs, preferences, and health conditions.Educate patients on nutrition, exercise, behavior modification techniques, and other strategies to promote healthy weight loss and sustainable lifestyle changes.Monitor patients’ progress closely through regular virtual follow-up visits, adjustments to treatment plans as needed, and ongoing support and encouragement.Utilize evidence-based practices and stay abreast of the latest developments in weight management and telehealth technology to enhance patient care and outcomes.Maintain accurate and thorough documentation of patient encounters, treatment plans, and progress notes in electronic health records (EHR) systems.Adhere to all relevant legal and regulatory requirements, including patient privacy and confidentiality guidelines, as well as organizational policies and protocols.
Qualifications:
- Graduate of an accredited Nurse Practitioner or Physician Assistant program.
Current certification and licensure as an NP or PA in the state of practice.Excellent communication and interpersonal skills, with the ability to establish rapport with patients and provide empathetic, nonjudgmental support.Experience or interest in weight management, nutrition counseling, and behavior change interventions preferred.Comfortable using telehealth platforms and technology to conduct virtual visits and communicate with patients remotely.Ability to work independently and collaboratively within a multidisciplinary team environment.Commitment to ongoing professional development and continuing education in the field of weight management and telehealth.
