Full Job Description

Summary

A licensed and credentialed Nurse Practitioner or Physician Assistant (NP/PA) provides diagnosis, treatment, consultation, and follow-up patient care under the supervision of one physician in the practice of Geriatrics in The Department of Medicine at Harris Health System within the House Calls Program.

The NP/PA will function as a primary care provider maintaining and promoting the health of the patient at times providing routine, urgent or overflow care.

The NP/PA provides age and specialty appropriate medical care at the level of training achieved conforming to the Texas Board of Nurse Examiners (BNE) or the Texas Board of Medical Examiners rules and regulations as well as all federal, state, or local laws, affecting the NP/PA’s current area of practice.

Job Duties

  • Effectively identifies, evaluates, and addresses acute and chronic problems as well as disease prevention and health promotion issues of the patients while administering quality patient care
  • Works in an independent and interdependent relationship with the physician, which allows for consultation, collaboration, or referral
  • Responsible for initial consultation and/or follow up diagnosis and treatment of acute and chronic healthcare issues of clinic patients
  • Responsible for communicating test results and recommendations to patients and/or their caregivers
  • Responsive to the questions and concerns of patients and their family members
  • Educates patients and/or families about preventative care, medical issues, and use of prescribed medical treatments and or medications
  • Maintains accurate and confidential medical records. Documents all medical evaluations, diagnoses, procedures, treatment, outcomes, education, referrals, and consultations consistent with state regulatory standards.
  • Facilitates evaluation of records by physicians and peers and receives and implements constructive directives.
  • Prepares cardiac clearance letters to referring physicians, for transplant and pre-operative clearance.
  • Prescribes medications in accordance with prescriptive authority granted by the Texas Medical Board/Board of Nurse Examiners.
  • Maintains professional certification as dictated by the Texas Medical Board/Board of Nurse Examiners.
  • Follows latest ACC and AHA guidelines as protocol and standard of care which shall be defined to promote the exercise of professional judgment by the NP/PA commensurate with his/her education and experience
  • Review billing cases log
  • Demonstrates a positive attitude in the workplace and is a leader in this regard.
  • Provides follow up patient care within a private practice setting.
  • Performs other duties as assigned.

Minimum Qualifications

For Nurse Practitioner:

  • Education Required: Master’s degree in Nursing.
  • Experience Required: Prior outpatient clinic experience.
  • Certification/Licenses/Registration: Current license as a Registered Nurse (RN) by the State of Texas Board of Nursing. Current Texas Licensure as an Advanced Practice Nurse (APRN-NP), and specialty certification as required by department. Basic Life Support (BLS) Certification

For Physician Assistant:

  • Education Required: Master’s degree in Physician Assistant Studies
  • Experience Required: Prior outpatient clinic experience.
  • Certification/Licenses/Registration: Current license as a Physician by the Texas Medical Board. Basic Life Support (BLS) Certification
